Job Description

This position is for 1 part time 30 hours per week post and 1 part time 22.5 hours per week post, some weekend working is required.
This is an exciting time to join our team, supporting us to care for patients safely in their own home avoiding unnecessary admissions and facilitating early discharges from hospital.
UCR/VW is a multi-disciplinary team that provides a two-hour urgent response to patients in their own home that are experiencing a crisis. The service can clinically monitor patients remotely via Virtual Ward using enhanced technology.
This role requires a highly organised person, with ability to prioritise their workload and work in a calm and effective manner and communicate well at all levels.
Main duties of the job
This role will involve managing the case load of clinical staff within the Urgent Community Response (UCR) and Virtual Ward (VW) Team. It will involve gathering, inputting and providing essential data for patients, helping to maintain accurate records and enhancing the team's ability to provide personal care.
The role is responsible for the administration support and caseload co-ordination within the team to support the delivery of care to patients. The post holder would also be required to answer the telephone, which may include calls from patients, and information then passed onto a clinician.

  • Maintain an accurate database of the home monitoring devices.
  • To be responsible for data collection, based on the outcomes from the clinical caseload.
