Summary



We are seeking a compassionate and skilled

Care Home Assistant Practitioner (CHAPS) QUALIFIED IN UK

to join our team at St Peters Court Nursing Home. In this role, you will play a vital part in delivering high-quality care to our residents, ensuring their comfort and dignity while supporting their individual needs. The successful candidate must be able to work night shifts.

To professionally lead the care team under the supervision of the Nurse on Duty, to ensure that a safe and effective care service is maintained which meets the needs of the residents. To train and supervise all members of the care team to work to the best of their ability and give the highest standard of care possible.

Responsibilities



As a Care Home Associate Practitioner, you will play a pivotal role in ensuring the highest standard of care delivery and staff development within our care home. Your duties will include, but are not limited to:

Training & Development

: Equip staff with the knowledge, practical skills, and motivation to perform their roles. Undertake the delivery of in-house training programs and collaborate with HR to identify training needs and develop professional growth.

Leadership & Supervision

: Provide leadership and guidance to the care team, ensuring that care is delivered according to the latest guidelines and best practices. Supervise day-to-day activities and conduct formal supervision with staff.

Resident Care & Safety

: Ensure the ongoing safety of residents, staff, and visitors. Monitor care delivery, conduct risk assessments, and maintain accurate documentation. Administer medication under the supervision of a registered nurse and provide personal care, including bathing, dressing, and grooming.

Clinical Observations

: Regularly monitor residents' health, including vital signs and skin integrity. Manage wound care and assist with diabetes management, catheter care, and other clinical observations as required.

Documentation & Reporting

: Maintain accurate records and communicate effectively regarding resident care. Send weekly reports to the Home Manager on staff documentation and monthly reports on care plans, risk assessments, and MCA compliance.

Team Collaboration

: Ensure effective handover systems between shifts, and support new Senior HCAs and care staff during their induction and competency assessments. Actively participate in team meetings and promote a positive work culture.

Health & Safety

: Promote excellent Health and Safety working practices and ensure all mandatory training is completed. Advocate for the residents' individual needs and promote dignity, equality, and diversity

MANDATORY REQUIREMENTS:-



. Excellent communication skill

. Care Home Assistant Practitioner (CHAPS) QUALIFIED IN UK

Worked in a Care Home setting for 12 months or more as a Senior carer or CHAP's

NVQ Level 3 (minimum)

Management skills

To be able to deliver direct training

Empathy and consideration to individuals and situations

Ability to work on own initiative in a fast-paced environment

. Ability to manage conflicting priorities in an effective and timely manner
