Job Description
Join our dedicated team in Cheshire East as a Qualified Social Worker (QSW) in Adult Social Care, where you will play a vital role in enhancing the quality of life for adults in our community. You will be responsible for assessing the needs of service users, developing care plans, and coordinating support services to ensure the best outcomes for individuals.
Your key responsibilities will include:
- Conducting thorough assessments of adults' social care needs.
- Developing and implementing personalized care plans in collaboration with service users and their families.
- Providing ongoing support and guidance to individuals and their caregivers.
- Working closely with multidisciplinary teams to ensure holistic care delivery.
- Maintaining accurate and up-to-date case records in compliance with legal and organizational standards.
- A recognized social work qualification (e.g., BA/MA in Social Work).
- Current registration with Social Work England.
- Proven experience in adult social care, including knowledge of relevant legislation and best practices.
- Excellent communication and interpersonal skills to effectively engage with service users and stakeholders.
- Strong organizational skills and the ability to manage a caseload effectively.
