Join our dedicated Children's Diabetes Team at Somerset NHS Foundation Trust, where you'll play a vital role in supporting children and young people with diabetes and their families. The team provides a cross-county service, and you will be based on the Children's Unit at Musgrove Park Hospital, delivering specialist advice, education, and care in hospital, community, and school settings.


This is an exciting opportunity for a registered children's nurse with experience in paediatrics and a passion for supporting children with diabetes. There is an option for this post to commence as a Band 5 development post, depending on your previous skills and experience.


You'll manage a defined caseload, support families in hospital, home, and school settings, and work closely with a multidisciplinary team. The role involves empowering families in diabetes management, supporting transition to adult services, and contributing to service development.


Travel is required, and you may also be asked to support services across the county. A car user allowance is provided.




Main duties, tasks & skills required

Deliver specialist nursing care, education, and support to children and young people with diabetes and their families. Manage a defined caseload, providing care in hospital, home, and school settings. Work collaboratively with a multidisciplinary team including paediatricians, dietitians, psychologists, youth workers, and administrators. Support transition planning from paediatric to adult diabetes services. Provide training and advice to hospital and community staff on diabetes management. Respond to urgent queries and provide clinical advice to prevent complications and hospital admissions. Maintain accurate clinical records and contribute to audits and service evaluations. Promote evidence-based practice and contribute to policy development.

As a Children's Diabetes Specialist Nurse, you will:

Conduct assessments and develop care plans tailored to each child's needs, including those with additional health or learning needs. Educate families on managing diabetes during everyday life and special circumstances (e.g. travel, social events, school trips). Provide guidance on insulin therapy, blood glucose monitoring, and specialist equipment such as insulin pumps. Deliver health education on risk-taking behaviours and lifestyle choices relevant to diabetes. Support families with disability benefit applications and provide professional reports as needed. Promote evidence-based practice and contribute to service development through audits, policy updates, and clinical supervision. Maintain clinical competence through ongoing training and professional development. Use Trust systems to manage outpatient activity and maintain statistical records of service delivery. Travel across the region to deliver care and attend meetings, with lone working as required.

This role requires a compassionate and proactive approach, with strong communication and teaching skills. You'll be joining a team whose purpose is to empower young people with diabetes to live healthy, happy, and fulfilling lives.

Qualifications

Essential




RSCN/RN Child.


Diploma in health care related subject.


Graduate or undertaking degree level study. Professional or clinical qualification relevant to the speciality (eg. Children's Diabetes Modules).

Desirable




Teaching qualification. Or willingness to undertake study to complete course.


Community nursing qualification (eg. health visitor, community children's nurse).


Experience

Essential




Post registration nursing experience.


Experience of caring for children with diabetes.

Desirable




Children's community nursing experience.


Experience of READY/STEADY/GO transition paperwork.


Experience with children with diabetes.
