Facilitate access to community resources, including housing, healthcare, and employment services.
Maintain accurate case records and documentation in accordance with organisational policies.
Collaborate with other professionals and agencies to coordinate care for clients.
Advocate for clients' rights and needs within the community.
Participate in training and professional development opportunities to enhance skills.
Requirements
Previous experience in case management or a similar role is advantageous.
Strong mentoring and supervising skills with the ability to motivate and support clients.
Excellent communication skills, both verbal and written, with an empathetic approach.
Ability to work independently as well as part of a team in a fast-paced environment.
Strong organisational skills with attention to detail in record keeping.
