Overview



We are seeking a dedicated and organised

Care Coordinator

to join our team. This role is central to ensuring the smooth delivery of high-quality, person-centred care. The Care Coordinator will act as the key link between service users, care staff, families, and healthcare professionals, ensuring that care plans are effectively implemented, monitored, and updated.

Key Responsibilities



Coordinate and schedule care staff to meet service users' needs. Develop, review, and maintain individual care plans tailored to each service user. Ensure compliance with CQC standards, safeguarding, and health & safety policies. Monitor the quality of care delivery through regular reviews, audits, and feedback. Act as the first point of contact for service users and their families, addressing queries and concerns promptly. Liaise with healthcare professionals, local authorities, and external agencies to ensure continuity of care. Maintain accurate records, documentation, and reports in line with regulatory requirements. Support recruitment, induction, supervision, and training of care staff. Participate in on-call rota as required.

Person Specification



Qualifications



Additional training in safeguarding, medication, or care planning is desirable.

Experience



Previous experience in a care coordination, senior care worker, or similar role. Strong understanding of domiciliary care or residential care services. Experience in staff supervision and rota management.

Excellent communication and interpersonal skills. Strong organisational and time-management abilities. Ability to work under pressure and problem-solve effectively. Compassionate, empathetic, and committed to person-centred care. IT literate with experience using care management software (desirable).
