We are seeking a dedicated Case Manager to join our growing team. In this role, you will develop and implement comprehensive therapy and care plans, manage complex cases and co-ordinate with clients, their families and a multidisciplinary team to ensure positive outcomes. This position is available on either a contracted or self-employed basis, offers competitive pay, mileage and expenses, and will combine both office-based and home working.
About the Role:
Develop and implement therapeutic and care plans that support the client's choices and goals
Manage a caseload of complex cases - including clients who have suffered traumatic brain injuries, , spinal cord injuries, complex orthopaedic injuries and cerebral palsy
Co-ordinate services and act as a liaison between the client, health providers and other stakeholders
Communicate sensitive information and provide support to clients and their families
Foster a culture of continuous improvement and teamwork
Bring joy and hope when people need it most
What You Will Need:
Registration as a Healthcare professional (e.g. a Registered Nurse, Physiotherapist, Occupational Therapist etc.)
Proven experience in coordinating therapy programmes and delivering outcomes in an efficient manner.
Excellent communication, leadership and interpersonal skills
A strong understanding of patient needs and the healthcare system
Previous experience of dealing with people who have suffered complex injuries or have a lack of capacity
