Job Description

Full job description attached Working at an advanced practice level to provide a co-ordinated and comprehensive specialist palliative care service to patients and their loved ones facing a terminal illness within the hospice catchment area. Working within a skill mixed team, be responsible for a caseload of community patients. This will include supporting appropriate identification and referral of patients, conducting comprehensive holistic assessments, diagnosis, recommendation of evidence-based symptom management plans and enable advance care planning within the care home setting. The post-holder will be an expert role model providing education and support regarding palliative care issues to other health and social care professionals and, in conjunction with senior clinical management, stimulate and direct the development of clinical practice across the local community.
