Clinical Nurse Specialist (cns) For Inclusion Health

Job Description

The Inclusion Health team is an exciting service as part of the Integrated Care Operations team at UCLH. The successful candidate will work alongside our Inclusion Health Consultant, another Inclusion Health CNS and our support worker.



The aim of the role is to focus on our Inclusion health group patients in hospital from working with the Emergency Department and inpatient wards across all UCLH sites. You will support patients from admission to discharge focusing on discharge planning, advocating for them and trying to integrate them into healthcare. You will also focus on the high intensity users of ED who fall under our Inclusion Health team and supporting the HIU team to reduce their ED attendances.



The role also focuses on supporting those people who are rough sleeping on site/outside UCLH. The successful person will work with on-site hospital teams and community partners such as our street teams and GPs.



There are lots of opportunities in this role which focuses on many aspects of our patients health including physical health, mental health and social support.



Our aim is to provide a safe and sustainable accommodation for our patients and clients where they can remain engaging with healthcare.



Reviewing patients on admission to ED/inpatient wards Supporting discharge - completing DTRs and liaising with community teams such as the local council, immigration services for legal advice Supporting hospital flow Escalating delays Holistic assessments of their healthcare and social needs Supporting high intensity users and the MDT in formulating a plan for them to reduce the attendances Providing education to the trust Street outreach in and around UCLH Working with outreach teams Supporting our consultant with medical assessments Attending MDTs Supporting improvement for our team and the trust Providing an advocacy service for our patients and clients Providing a high standard of clinical assessments for our patients
