Job Description

Provide clinical support and expert care to patients with a haematological malignancy at ASPH throughout their cancer journey.



Be the key worker for patients, families and carers with a haematological malignancy



Support the faster diagnosis and improve outcomes for patients with haematological malignancies



Positively impact on the experience of patients and carers by providing high standards of clinical practice and care as appropriate across the trust



Support the delivery of a haematological diagnosis



Deliver HNA/care plan clinics for all patients within 6 weeks of a confirmed malignant diagnosis



Assess patients pre chemotherapy and deliver pre-chemotherapy information



Support patients and carers though-out their cancer journey, provide support and advice to patients calling in though the single point of contact number.



Be prepared to implement and support PSFU for lymphoma
