Job Description

An exciting opportunity has arisen within our Prostate Cancer team for a energetic and enthusiastic Clinical Nurse Specialist.



This opportunity will look to support our established Self Supported Management programme designed to enable patients to manage consequences of cancer during treatment and beyond, by providing clinical expertise and guidance



The Prostate Cancer Clinical Nurse Specialist will provide expert leadership and specialist nursing care for patients living with and beyond prostate cancer. As professional lead for the Self-Supported Management (SSM) programme, the CNS will:



Specialist Clinical Care:

Deliver advanced assessment, support, and evidence-based interventions for patients across the prostate cancer pathway, addressing physical, emotional, and social needs.



SSM Programme Leadership:

Lead the design, delivery, and evaluation of the SSM programme, supporting patient empowerment, personalised care planning, and long-term self-management.



Patient and Family Education:

Provide clear information and guidance to promote self-care, shared decision-making, and improved quality of life.



Professional Leadership:

Act as a role model, mentor, and educator for nursing colleagues and the wider multidisciplinary team, ensuring high standards of prostate cancer care.



Service Improvement:

Use patient outcomes, audit, and feedback to evaluate and improve the SSM programme and wider prostate cancer services.



Collaborative Working:

Build strong links across hospital and community teams to ensure smooth care transitions and coordinated support.



Governance and Safety:

Support clinical governance processes to maintain safe, effective, and patient-centred practice.
