Clinical Pharmacist Bidford Health Centre - Part time 22.5-27 hours per week

Bidford Health Centre, located near the semi - rural village of Bidford on Avon and within easy driving distance of Stratford-upon-Avon, Alcester and Evesham for example and is a large, modern and purpose built health centre incorporating a dispensary and is a teaching practice for GPs, nurses , Physician Associates and medical students. We have a list size of 14,250 patients and an exciting future.

We have a strong team of 4 Partners, 5 Salaried GPs, Nursing team of 7 and multi-disciplinary team of clinical and non-clinical staff including 2 other practice based Clinical Pharmacists.

You will join an excellent team of friendly, caring, experienced and capable clinicians.

KEY DUTIES AND RESPONSIBILITIES

Perform medication reviews of patients Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care Improve patient and carer understanding of, confidence in and compliance with their medicines. Reconcile medicines following discharge from hospitals, intermediate care and into care homes; identify and rectify unexplained changes; manage changes by referral to the GP Support the processing of complex repeat prescribing medication requests made by patients or other repeat prescribing queries.

Accessing the health centre is easy by car with plentiful parking and a bus service outside running between local towns. We expect the role to be practice based as an integral part of our MDT.

The successful applicant should have completed CPPE pathway and need to complete IP (if not already achieved) a DBS assessment and usual checks as part of the recruitment process. You will benefit from a good holiday entitlement, access to the NHS pension and an excellent working environment.

Pay: 42,546.87-50,000.00 per year
