Job Overview

We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in optimising patient care through the safe and effective use of medications. This position requires a strong understanding of pharmacotherapy, patient assessment, and the ability to work collaboratively with healthcare professionals to ensure the best outcomes for patients.

Duties

Conduct comprehensive medication reviews and assessments for patients, ensuring appropriate medication therapy management. Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualised medication plans. Provide patient education on medication usage, potential side effects, and adherence strategies. Monitor patient responses to medications, adjusting therapy as necessary based on clinical judgement and evidence-based guidelines. Maintain accurate and up-to-date patient records regarding medication administration and therapeutic outcomes. Stay informed about new medications, therapies, and clinical practices to enhance patient care.

Skills

Proficient in
