Job Description
Provide clinical assessment and management of patients on anticoagulation therapy (including initiation, monitoring, and dose adjustment). Prescribe and optimize anticoagulation treatment in line with local and national guidelines. Conduct patient consultations, review INR results, and ensure timely follow-up. Maintain accurate clinical records and contribute to audit and quality improvement initiatives.
Collaborate with GPs, nurses, and other healthcare professionals to ensure seamless patient care. Support education and counselling for patients regarding anticoagulation therapy and adherence.
