East Basildon PCN is a Primary Care Network and are seeking a clinical pharmacist to join their team on a full-time basis, working Monday to Friday across 4 GP practices. We have an exciting opportunity for a pharmacist to join our well-established clinical practice pharmacy team, to work across our 4 clinical patient facing sites including Felmores Medical Centre, Aryan Medical Centre, Matching Green Surgery and Dr Sims and Partners.
What's the role?
You'll work with a highly-skilled, well supported and forward thinking clinical team.
Essentially you will provide support for the management and development of prescribing systems to ensure safe, effective and quality prescribing within unity.
You will be a key member of the clinical pharmacy practice team working alongside pharmacists and pharmacy technicians reporting to the senior pharmacist.
You will also be involved in processing Accurx queries from patients.
Main responsibilities will include;
Meet patients with single or multiple long term medical problems where medicine optimisation is required.
Review the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e. medicines optimisation).
Make appropriate recommendations to the team for improvement in care.
Undertake clinical medication reviews with patients and produce recommendations for the clinical team on prescribing and monitoring.

Risk stratification Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches. This might include risks that are patient related, medicine related, or both.
Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups.
Manage caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence. Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate.
Provide patient facing clinics for those with questions, queries and concerns about their medicines in the practice.
Provide a telephone help line for patients with questions, queries and concerns about their medicines.
