We are seeking an enthusiastic and motivated Clinical Pharmacist to join our expanding multidisciplinary team. This is an excellent opportunity to play a key role in improving medicines optimisation and patient outcomes across primary care.
The Role
The post will align with the NHS 2025/26 Network Contract DES and will involve delivering high-quality clinical pharmacy services across the PCN.
Key duties and responsibilities include:
Conducting structured medication reviews (SMRs), particularly for patients with:
Complex polypharmacy
Frailty
Long-term conditions
High-risk medicines
Providing clinical medication advice to the practice teams and wider PCN MDT
Supporting medicines optimisation, including deprescribing and improving cost-effective prescribing
Managing medication-related queries, medicines reconciliation, and repeat prescribing processes
Supporting long-term condition management, such as hypertension, diabetes, respiratory disease, and cardiovascular disease
Identifying and managing patients at risk of medicines-related harm
Supporting care home medication reviews where required
Contributing to clinical audits, QOF, IIF, and PCN priorities
Working collaboratively with GPs, nurses, ARRS staff, and community pharmacy colleagues
Supporting quality improvement, medicines safety, and prescribing governance
Undertaking patient-facing and non-patient-facing clinical work in line with PCN objectives
Person Specification
Essential:
GPhC-registered pharmacist
Experience working in primary care, general practice, or community pharmacy with clinical responsibilities
Strong clinical decision-making and communication skills
Ability to work autonomously and as part of a multidisciplinary team
Desirable:
Independent Prescriber qualification (or willingness to train)
Experience delivering Structured Medication Reviews
Knowledge of PCN DES and ARRS roles
Experience working across multiple GP practices
