Clinical Pharmacist -

Role Summary:

We are seeking a proactive and experienced Clinical Pharmacist to join our multidisciplinary team. The successful candidate will play a key role in medicines optimisation, patient safety, and continuity of care, particularly in the context of long-term condition (LTC) management and polypharmacy.

Key Responsibilities:

Conduct comprehensive polypharmacy reviews to ensure safe, effective, and evidence-based prescribing.

Identify and implement alternative medications when standard options are unavailable, including liaison with specialists as needed.

Adjust prescriptions to alternative formulations (e.g., liquid forms) to meet patient-specific needs.

Provide cover for LTC pharmacist duties during periods of leave, including:

- Reviewing incoming blood test results related to LTC reviews.

- Actioning results in line with practice protocols and clinical guidelines.

Essential Skills & Attributes:

Strong clinical knowledge and experience in primary care pharmacy.

Excellent communication and collaboration skills.

Ability to work independently and manage clinical priorities.

Familiarity with EMIS or similar clinical systems is desirable.
