The Clinical Pharmacist role will provide a fantastic opportunity for a driven and passionate individual to drive innovation and support the ongoing excellent care of patients in the Bradford area. The Role is supported by a consultant led MDT. Experience with working with Frail older adults is an advantage. Patients are in their own homes, discharged early from hospital, or their admission to hospital has been avoided and they require support worth ongoing medical, and therapy needs
Main duties of the job
The post is an integral part of the multidisciplinary team, responsible for supporting patients in their own home in medication management and level 3 medication reviews. The post holder will direct the MDT in medication review/decision making will provide technical support to patients in their own home and work in liaising between the acute base interfaces with primary care. The post holder will work in a blurred boundary was, delivering some set skills core to nursing and therapy.
