Clinical Duties Conduct clinical & structured medication reviews Manage patients with chronic diseases (e.g., hypertension, Type 2 diabetes, asthma, COPD) Support polypharmacy reviews, frailty and those with complex comorbidities Provide expert medicines advice and respond to clinical queries Support safe prescribing systems, acute prescription requests, repeat prescriptions and medicines reconciliation Use EMIS Web and ICB templates to support safe clinical practice Provide leadership on medicines optimisation, antimicrobial stewardship, QOF and enhanced services Support quality improvement, clinical audits, and prescribing safety initiatives The right candidate will be supported to further their education and qualifications and develop areas of special interest / expertise Practice Development Work Improve QOF domains and disease registers Lead on prescription management processes Work closely with local pharmacies and admin staff to ensure safe systems and monitoring are maintained.
