Support Pharmacy Technician Clinical medication review Patient-facing clinics for people living with long-term conditions Receive on-the-day referrals for patients needing advice and guidance Management of common/minor/self-limiting ailments Provide medicine information to practice staff and patients Signposting Repeat prescribing Offer support, education and training to others Reducing unplanned hospital admissions Management of medicines after discharge from hospital Safety netting Risk stratification Information management and service development Medicines quality improvement and medicines safety Undertake clinical audits of prescribing in areas to ensure compliant with CQC standards Communicate key updates to Practice colleagues Support Practice Systems for recording concerns Support implementation of local and national guidelines and formulary recommendations WORKING RELATIONSHIPS All Burwell Surgery staff, PCN and community teams, suppliers and patients.
