Are you an enthusiastic and forwards thinking qualified Clinical Pharmacist?

If so this is an exciting opportunity for you to join our existing pharmacy team, working with general practices across our PCN footprint, with support from our existing clinical teams to deliver excellent patient care. You will be part of our growing multi-disciplinary additional roles team, who all work together to support our GP practices and their patients.The roles currently include Care Coordinators, Clinical Pharmacists, Pharmacy Technicians, Paramedics, Health and Wellbeing Coaches, Social Prescribing Link Worker, Outreach Nurses and Mental Health Practitioner.

Main duties of the job



The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face-to-face structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, acute prescription request, while addressing both the public health and social care needs of patients in the GP practice(s) that make up the PCN.

The base for this role will be at Henry Moore Clinic, but working with all six practices within the PCN.

Wakefield North Primary Care Network (PCN) are developing our services to support our six general practices and patients in our PCN across the Eastern side of Wakefield District, serving a population of approximately 48,700. Our practices are;

Job description



Job responsibilities



The following are the core responsibilities of the PCN Clinical Pharmacist, with the requirement, on occasion, to carry out other tasks. This will be dependent on factors such as workload and staff levels:

Undertake level 3 medication reviews to support improved patient outcomes. Undertake Structured medication reviews for patients who are highlighted in the Network DES. Review and issue acute medicine requests. Carry out medicine reconciliations of hospital discharge/clinic letters. Improve medicines optimisation by reviewing the on-going need for each medicine. Support Practices to achieve QOF targets as required. To provide advice to GPs, Nurse Practitioners, Practice Nurses, Community Pharmacists, Community Nurses, and other health professionals to promote the safe, effective, high-quality, cost-effective use of medicines in all prescribing decisions for the benefit of the Practice's patients. Contribute to the improvement of health outcomes in long-term conditions as part of the multidisciplinary team. Patient facing care home medication reviews, working with care home staff to improve safety of medicines ordering and administration. To provide prescribing advice and support to the practice(s), on all medication issues. Accurate and consistent recording and monitoring of data including incident reporting, prescribing data, and patient outcomes. Work in partnership with GP practices and the ICB /ICS Ensure that all major MHRA drug alerts, where appropriate and within levels of competence, are acted upon within practices over an appropriate time frame and advise on any medication changes that occur as a result as instructed by the ICB Medicines safety Officer. Provide peer support to other clinical pharmacists and pharmacy technicians in the PCN. Review the use of medicines most commonly associated with unplanned hospital admissions and re-admissions through audit and individual patient reviews. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups. Undertake clinical audits of prescribing in areas directed by the GPs, feedback the results and implement changes in conjunction with the relevant practice team.

Person Specification



Qualifications



Essential



Master's degree in pharmacy or equivalent Registration with the General Pharmaceutical Council (GPhC). Evidence of Continued Professional Development Independent Prescriber or willing to work towards Completion of the Primary care pharmacy education (CPPE) pathway or willing to work towards

Desirable



Postgraduate Diploma in Clinical Pharmacy (or equivalent)
