Duties

Review and interpret prescriptions to ensure accuracy and appropriateness for individual patients Provide expert advice on medication selection, dosage, interactions, and side effects Collaborate with healthcare professionals to develop and implement patient-specific treatment plans Monitor patient responses to medication therapy and make necessary adjustments Educate patients on medication usage, potential side effects, and adherence strategies Maintain accurate documentation of all pharmaceutical interventions and patient interactions Ensure compliance with legal, ethical, and organisational standards related to pharmacy practice Participate in multidisciplinary team meetings to support comprehensive patient care Assist in the development of clinical protocols and pharmacy policies

Requirements

Qualified Pharmacist with registration recognised by the relevant professional body Proven experience in a clinical setting, preferably within an ICU environment Strong knowledge of human anatomy, physiology, and pharmacology Demonstrated expertise in medication administration and patient care procedures Excellent communication skills with the ability to educate both patients and healthcare colleagues effectively Ability to work collaboratively within a multidisciplinary team Commitment to continuous professional development and staying current with pharmaceutical advancements Strong organisational skills with meticulous attention to detail
