The Clinical Pharmacist will work as an integral member of the practice clinical team, with a strong emphasis on
direct patient care
,
medication reviews
, and
long-term condition management
. The role involves close collaboration with GPs, nurses, and other healthcare professionals to ensure safe, effective, and evidence-based prescribing.
Key Responsibilities
Patient Care & Medication Optimisation
Conduct structured medication reviews, including for patients with complex needs
Provide patient-facing clinics (face-to-face, telephone, and remote)
Support medicines optimisation to improve safety, adherence, and clinical outcomes
Manage medication-related queries from patients and clinicians
Support de-prescribing where appropriate, in line with national and local guidance
Action drug alerts in a timely manner
Ensuring all medication related policies and procedures are up to date within the practice
Long-Term Condition Management
Support management of chronic diseases such as hypertension, diabetes, asthma, COPD, and cardiovascular disease
Work with the nursing team to support care planning and treatment optimisation
Monitor and interpret clinical results related to medicines management
Prescribing & Clinical Support
Prescribe independently (where qualified) within scope of practice
Support repeat prescribing processes and reduce prescribing workload for GPs
Review and action hospital discharge summaries and medication changes
Contribute to audits, QOF, LCS and prescribing quality initiatives
Ensure safe prescribing in line with CQC
Support the transfer of new patients ensuring medicines are reviewed and updated as necessary
Reporting of annual CD declaration and any relevant CD related incidents
Participation in shared care
Supervision of high risk drug monitoring processes
Multidisciplinary Team Working
Work closely with GPs, nurses, HCAs, and the wider clinical team as well as care home staff, district nurses, community nurses and PCN colleagues
Work closely with pharmacists and pharmacy technicians who support the practice as part of the Viaduct Team
Provide medicines-related advice and education to the wider team
Participate in practice meetings and service development
Support training of medical and nursing students and trainee doctors within the practice
Liaise with secondary care colleagues when needed for support and queries
Support non clinical staff within the practice in relation to medication queries and working within local policies and procedures
Governance & Professional Development
Maintain accurate and timely clinical records
Work in line with NICE guidance, local formularies, and best practice
Engage in ongoing professional development and clinical supervision
Job Type: Full-time
Pay: 45,000.00-55,000.00 per year
