Overview


We are seeking a dedicated and experienced Clinical Pharmacist to work as part of a multidisciplinary team at the Practice. The successful candidate will play a vital role in providing expert medication management and patient care within a clinical setting. This position offers an opportunity to work in a dynamic environment, supporting patient safety and treatment efficacy through specialised pharmaceutical services.

The successful candidate will provide clinical leadership for medicines optimisation, undertake clinical medical reviews, whilst supporting and advising patients with chronic disease management issues.

The role includes working closely with our GPs and other clinical staff to help to manage long-term conditions, advising those taking multiple medicines (polypharmacy) and delivering clinical advice about treatments. You will also assist with communication across a patient's care pathway, manage medicines shortages by suggesting suitable alternatives where appropriate.

Duties



Provide expert advice on medication usage, potential side effects, and interactions to both patients and healthcare teams Proactively seek to reduce medicine-related hospital admissions and readmissions by identifying medicine-related issues and supporting patients to get the best outcomes from their medicines Perform regular patient reviews to identify and address any medicines related issues and ensure patients get the best outcome from medicines prescribed Make appropriate recommendations to GP and other healthcare professionals for patents' ongoing treatment and for the management of referrals Research and identify patients who require medication reviews, including patients' clinical condition, blood monitoring and care arrangements Ensuring repeat authorisation processes are adhered to for high-risk medicines Undertake and deal with minor ailments and triaging of patients Maintain detailed records of medication dispensed and patient interactions in accordance with regulatory standards Participate in multidisciplinary team meetings to optimise patient care strategies Educate patients on medication adherence, lifestyle modifications, and health management practices Managing, auditing and reviewing prescription and repeat prescription policies to improve the quality, safety and cost-effectiveness of prescribing. Reduce wasteful polypharmacy and seek to reduce inappropriate and wasteful prescribing of medicines

Experience and qualifications



Proven experience working as a Pharmacist within a clinical or hospital environment. Strong knowledge of anatomy, physiology, and pharmacology principles relevant to patient care Demonstrated ability to administer medications accurately and safely in high-pressure situations Experience in providing comprehensive patient care, including counselling and medication management Familiarity with healthcare software systems for prescription processing and record keeping Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council Minimum of 2 years post-graduate experience in pharmacy Member of the Royal Pharmaceutical Society (RPS) Hold a prescribing qualification Member of or working towards (through foundation) RPS Faculty membership

Pay: Up to 62,000.00 per year
