About the Role

We are seeking a dedicated and motivated

Clinical Pharmacist

to join our friendly GP surgery team in Horsham. This is a

full-time, permanent role

where you will play a vital part in delivering safe, effective, and patient-focused medicines management within primary care.

The successful candidate will either be

already enrolled on the CPPE Primary Care Pharmacy Education Pathway (PCPEP)

or be willing to undertake it as part of their role.

Key Responsibilities

Provide structured medication reviews for patients with polypharmacy, frailty, and long-term conditions. Support prescribing safety and optimise medicines management across the practice. Deliver specialist clinics (e.g., hypertension, asthma, diabetes) as appropriate. Work closely with GPs, nurses, ANPs, and other healthcare professionals to improve patient outcomes. Respond to medicine-related queries from patients and colleagues. Contribute to repeat prescribing processes, audits, and prescribing initiatives. Support QOF, IIF, and other quality improvement schemes.

About You

Qualified Clinical Pharmacist with GPhC registration. Either: Already enrolled on the

CPPE Primary Care Pharmacy Education Pathway (PCPEP)

, or Willing to enrol as part of this role. Experience in primary care or a strong interest in developing within this setting. Excellent clinical decision-making, communication, and organisational skills. A proactive, patient-centred approach to medicines management.
