The clinical pharmacist will be supported and mentored by our senior pharmacist and Clinical Partners. You will have regular contact with patients and will reduce the workload of GP prescribers. Your duties will include: Remote and patient facing structured medication reviews Management of long-term conditions within scope of practice Support the pharmacy technicians and prescription clerks manage acute and repeat prescription requests Telephone medicines support for patients with questions, queries and concerns about their medicines Medicine's information to practice staff and patients Management of medicines at discharge from other places of care: hospital/care homes Driving appropriate use of e- Repeat Dispensing Delivering any quality improvement activity for prescribing required by QOF or any other contractual agreements made by the PCN Contribute pharmaceutical advice for the development and implementation of new services Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance. Provide education and training to primary healthcare team on therapeutics and medicines optimisation.
