Title: Clinical Pharmacist
Responsible to: The Partners
Location: Weavers Medical Main Site and Mawsley Village Branch site
Hours per week: 25 hours per week spread over M-F (2 posts)
Main Purpose:
o
Support the repeat prescribing process; reauthorising medication as required, issuing acute prescriptions as required and managing medication queries, all in line with the practice prescribing standard operating procedure (SOP).
o
Undertake structured medication reviews for patients with polypharmacy, frailty, or complex care needs.
o
Undertake structured medication reviews for patients who have attended for a PCSP review appointment.
o
Support the patient safety officers to ensure the medication monitoring system is robust.
o
Review and manage hospital discharge and outpatient clinic letters where medication is changed, issued or stopped.
o
Work with the admin lead for patient safety alerts, ensuring all required searches and actions are undertaken and information is disseminated to relevant teams as required.
Key Tasks:
Repeat Prescribing Process
a)
Working from tasks and in line with the prescribing SOP, reauthorise and issue medication when it has been requested by patients.
b)
Working from tasks and in line with the prescribing SOP, issue acute medication when it has been requested by patients.
c)
Working from tasks and in line with the prescribing SOP, issue-controlled drug medication when it has been requested by patients.
Structured Medication Reviews
a)
Undertake SMRs as directed by the clinical lead, for patients with polypharmacy, frailty, and/or complex needs.
b)
Working from tasks, undertake SMR after PCSP, for patients who have attend a primary care support planning (PCSP/LTC) appointment.
c)
Undertake SMRs as a desktop review, telephone or face to face appointment (to be directed by the partner clinical lead).
Safety Officer - Medication Monitoring
a)
Work closely with the safety officer team to ensure all medication monitoring searches and recalls are up to date. Liaise with the partner clinical lead about any backlogs of work.

b)
Work closely with the partner clinical lead to identify ARDENs searches to run and monitor, to help improve the safety and quality of prescribing.
Hospital Discharge Letters
a)
Working from tasks, process hospital discharges and other clinic letters where medication needs to be changed, issue or stopped.
b)
Contact patients via telephone or Accurx as required and utilise the opportunity to realign medications and undertake SMRs as appropriate.
c)
Report any hospital prescribing anomalies or significant events to the partner clinical lead and via the significant event reporting system.
Safety Alerts
a)
Work with the admin lead for safety alerts to ensure each alert is actioned appropriately.
b)
Liaise with the safety team and management team for any follow up searches, recalls or other work required.
c)
Disseminate information to the various teams via email and team meetings.
Other Admin Tasks
a)
Attend prescribing meetings as the practice representative, as required from time to time.
b)
Process any medication related significant events, investigating what happened, adding to the significant event, then reporting back to the partner clinical lead.
c)
Provide expert clinical advice on medicines management and optimisation as required.
d)
Meet with patients face to face and issue medication contracts as required from time to time.
e)
Any other admin duties as considered relevant to the post
