We have been impressed by the impact that the Pharmacists and Pharmacy Technicians have had across Lancing and Sompting PCN and would like to expand our team's capacity here at Ball Tree surgery.
The population is growing, though it currently stands at 15,200 here at Ball Tree. At Ball Tree we currently have 2 Pharmacists, 2 Pharmacy Technicians and a pharmacy administrator that work closely within the practice teams and the wider PCN teams.
Our surgery strives to provide our patients with excellent Clinical Care and motivational support delivered by warm and caring clinical staff. We have excellent relationships with the local community pharmacies and the community teams.
Specialist interests, experience and independent prescribing qualifications are welcomed, though not essential, especially around chronic disease management including frailty and care homes. An interest in hypertension and lipid management as well as HRT would be beneficial.

Main duties of the job
The Clinical Pharmacist will be responsible for a combination of patient facing work such as medication reviews, chronic disease management, prescription queries and management of medicines through transfer of care. Working across two sites, they will also play a significant role in service and quality improvement through audits of relevant work processes, sharing of best practice and current guidance with regard to prescribing and through work in medicines optimisation.
The successful candidate will be supported in terms of clinical learning and professional development through regular access to a GP clinical supervisor, other clinical pharmacists and through a formal education pathway through the Centre of Postgraduate Pharmacy Education. As part of a growing team of Clinical Pharmacists there is also a programme of peer support events and team meetings in place.

Ball Tree surgery is a practice with 15,200 patients using EMIS across 2 sites. We are a friendly team with a great mix of clinical roles supported by a large team of administrators, secretaries, receptionists and care coordinators.
We have regular clinical meetings and education sessions to help with developing continued learning and improvement.
Job responsibilities
Clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
Be prescribers, or committed to completing training to become prescribers.
Contribute to the delivery of effective daily processes for the transfer of care between hospitals and GPs including effective signposting, recommendation of medication changes and communication accordingly.
Support colleagues and patients with medication queries and monitoring.
Take responsibility for the care management of patients with chronic diseases.
Undertake clinical medication reviews to proactively manage people with complex polypharmacy.
Focus especially on the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, hypertension, COPD and asthma) and people with learning disabilities or autism.
Provide specialist expertise in the use of medicines, while helping to address both the public health and social care needs of patients.
Through the use of audit and action planning, help to tackle inequalities.
Provide leadership on person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement.
Contribute to the General Practice quality and outcomes framework (QOF) and enhanced services.
Use structured medication reviews to support patients to take their medications to get the best from them, reduce waste and promote self-care.
Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets).
Work with general practice teams to ensure that practices are compliant with CQC standards where medicines are involved.
Take a lead role in some aspects of supporting further integration of General Practice with other healthcare teams to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
Specifically, contribute to the Quality Improvement Schemes (QIS) for practices (e.g. to reduce prescribing spend and unplanned admissions)
Develop relationships and work closely with other pharmacy professionals across Primary Care Networks and the wider health and social care system
Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, liaison with community pharmacists and anti-coagulation.
