The Role
We are recruiting a
full-time Salaried Clinical Pharmacist
to deliver high-quality medicines optimisation and direct patient care. This is a permanent position with an immediate start available.
Key Responsibilities
Conduct structured medication reviews for polypharmacy, high-risk patients, and care-home residents
Manage repeat prescribing systems, improve safety, and reduce waste
Support long-term condition clinics (e.g., hypertension, asthma, diabetes)
Reconcile medicines post-discharge and respond to medication queries from patients and staff
Lead quality improvement projects, audits, and implement local/national guidelines
Contribute to PCN pharmacy initiatives and prescribing incentive schemes
Supervise pharmacy technicians and support trainee pharmacists where applicable

for accurate clinical documentation
Working Pattern
Full time: 37.5 hours per week
(Monday-Friday)
Flexible start/finish times (core hours 8am-6:30pm)
No weekend or bank holiday working
30-minute paid lunch break daily
Protected CPD time (minimum 1 session per month)
Salary & Benefits
40,000-45,000 per annum
(dependent on experience)

Person SpecificationEssential
MPharm or equivalent + GPhC registration
Minimum
1 year post-registration experience
in primary or community care
Independent Prescriber (or willing to complete within 12 months - funded)
Strong clinical knowledge of chronic disease management and polypharmacy
Excellent communication, IT, and teamwork skills
Desirable
CPPE Primary Care Pharmacy Education Pathway (or equivalent)
Experience with SystmOne
Audit or quality improvement project leadership
Interest in care homes or frailty
