We are looking for a motivated and compassionate Clinical Pharmacist to join West Green Surgery N8, Haringey, North London. This is an excellent opportunity to contribute to high-quality patient care within a supportive general practice environment. You'll work closely with GPs, pharmacists, other healthcare professionals and administrative staff to improve medicines optimisation, support long-term condition management, and enhance patient outcomes.
Responsibilities
Review and interpret medication orders to ensure appropriateness for patient therapy.
Provide expert advice on medication therapy management and potential drug interactions.
Conduct medication reconciliation for patients upon admission, transfer, and discharge.
Monitor patient responses to medications and adjust therapies as necessary in collaboration with the healthcare team.
Educate patients and their families about medications, including proper administration and potential side effects.
Participate in clinical rounds within the ICU, providing insights on pharmacotherapy to optimise patient treatment plans.
Maintain accurate records of medication usage and contribute to quality improvement initiatives related to medication safety.
Stay updated with current pharmacological research and guidelines to enhance clinical practice.
Qualifications
A degree in Pharmacy from an accredited institution and valid registration with the relevant pharmacy council.
Strong knowledge of anatomy and physiology as it relates to pharmacotherapy.
Experience working in an ICU or similar high-acuity environment is highly desirable.
Proficiency in medication administration techniques and patient care practices.
Excellent communication skills, both verbal and written, with the ability to work collaboratively within a multidisciplinary team.
Strong analytical skills with attention to detail in reviewing medical records and prescriptions.
