To be responsible for implementing an effective medicine management system within the practice, including cost-effective prescribing, providing advice and support for both patients and colleagues.
Primary Responsibilities
The following are the core responsibilities of the clinical pharmacist. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
To provide medication review services with patients in the practice either face to face or on the phone
Encourage cost-effective prescribing within the practice
Review all safety alerts ensuring the required actions are taken
To reconcile medicines following patients discharge from hospital or clinic appointments
Implement and embed a robust repeat prescribing system
To provide subject matter expertise on medication monitoring, implementing and embedding a system
Carry out targeted medication reviews for high-risk patients
To re-authorise medication for repeat prescribing within scope of practice
Identify patients suitable for repeat prescribing, liaising with the relevant clinicians as required
Review and implement safe prescribing systems for vulnerable patients, improving compliance
Support clinicians with the management of patients suffering from drug and alcohol dependencies
Assist with QOF targets
Carry out CQC searches
Partake in audits as directed
Provide education on medicines for the clinical team
Review the latest guidance ensuring the practice conforms to NICE, CQC etc.
Act as the practice point of contact for all medicines related matters, establishing positive working relationships
Assist with EPS queries when pharmacy technician on leave
Person Specification - Essential
GPhC Registered Pharmacist
Masters Degree in Pharmacy
Minimum of 2 years post qualification experience
In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
An appreciation of primary care prescribing
Can plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
Recognises priorities when problem solving and can refer to GPs when appropriate
Can communicate complex and sensitive information in an understandable form to a variety of audiences (eg patients)
Excellent interpersonal, influencing and negotiating skills
Work effectively independently as well as in a team
Strong IT skills
Good problem solving and analytical skills
Ability to follow policy and procedure
Able to work under pressure and meet deadlines.
Flexible and cooperative
Self-motivated
High levels of integrity and loyalty

These requirements may be amended following consultation with the post holder, to facilitate the development of the role, the practice and the individual. All personnel should be prepared to accept additional, or surrender existing duties, to enable the efficient running of the practice.
