Overview

We are a friendly, supportive team of 8 doctors and 3 nurse practitioners looking to employ a dedicated and experienced Clinical Pharmacist.

Duties:

Carrying out medication reviews, including new patient medication reviews and structured medication reviews

Advising patients on prescriptions

Optimising prescribing

Medication reconciliation, reauthorisation and outpatient scripts

Liaising with GPs and ANPs

Supporting the duty GP with urgent prescriptions enquiries

Acting as the first point of contact for prescription-related queries

Supporting with audits, PIS, QIP projects, and CQC-related audit work

Working with the Prescription Clerks and supporting them with queries and medicines management

Assisting with medicines compliance and quality audits

Liaising with community pharmacies regarding stock and supplies

Supporting Prescription Clerks in initiating or amending medications from secondary care

Providing clinical management/support for the prescription team

Assisting in the development of prescribing processes and protocols

Running chronic disease clinics

Supporting us as a training practice with training and teaching as needed

Essential Experience:

Registered with the General Pharmaceutical Council (GPhC)

Strong knowledge of pharmacy legislation, GPhC standards, and professional ethics

Experience in patient counselling

Must be able to work independently, prioritise tasks, and manage multiple responsibilities

Desirable Experience:

To have completed the CPPE pathway for the Primary Care Pharmacy Education pathway, or be willing to undertake this

To have a prescribing qualification

GP Practice experience

Pay: From 48,500.00 per year
