The New Malden & Worcester Park PCN have a exciting opportunity for a Clinical Pharmacist to join our dynamic multidisciplinary team. This patient-facing role is to enhance the quality of care for our patients. The successful candidate will be provided with excellent clinical support and an opportunity to develop their primary care skills.

We are looking for motivated, team-orientated individual who shares our enthusiasm for improving patient care. Previous primary care experience is preferred as well as excellent verbal communication skills, a proactive approach and a desire to learn. As the role develops, you will take responsibility for areas of chronic disease management and undertake clinical medication reviews to proactively manage patients with complex poly-pharmacy. You will also provide primary support to general practice staff with regards to prescription and medication queries.

Main duties of the job

The post holder will work as part of a multidisciplinary team to provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing as well as manage repeat prescription authorisation and reauthorisations.

Job responsibilities

Patient facing long term condition clinics - Carry out consultations with patients with single or multiple medical conditions where medicine optimisation is preferred,

Review the on-going need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicine taking ensuring they get the best use of their medicines. Make appropriate recommendations to GPs for medicines improvement.

Patient facing clinical medication review - undertake clinical medication reviews with patients and produce recommendations for GPs on prescribing/monitoring.

Patient facing Care Home Medication Review - undertake clinical medication reviews with patients and produce recommendations for GPs on prescribing and monitoring. Work with care home staff to improve safety and appropriate medication ordering (medication waste)

Patient facing domiciliary clinical medication review - undertake clinical medication reviews with patients and produce recommendations for the senior clinical pharmacist, nurses and GPs on prescribing and monitoring. Attend and refer patients to multidisciplinary case conference

Management of common/minor/self-limiting ailments - Managing caseload of patients with common/minor/self-limiting ailments while working within scope of practice and limits of competence. Sign posting to community pharmacy and referring to appropriate healthcare professional i.e., GP/Practice Nurse/ Advanced Nurse Practitioner

Telephone medication support - provide telephone support for patients with queries or concerns about their medication

Management of medications at discharge from hospital - to reconcile medications upon discharge from hospitals, immediate care and into care homes including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge. Set up and manage systems to ensure continuity of medication supplies to high-risk groups of patients.

Medicines information to practice staff and patients - answer relevant medication queries from GPs, other network staff, other healthcare teams (e.g., community pharmacy) and patients with queries about medications. Suggest and recommend solutions. Provide follow up with patients to monitor the effect of any changes

Unplanned hospital admissions - Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups.

Signposting - ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period of time e.g., pathology results, common/minor ailments, acute conditions, long term condition review etc.

Repeat Prescribing - adhere and work within each network's repeat prescribing policy. Manage the repeat prescribing and reauthorisation process by reviewing patient requests for repeat medications and review medication review dates and arranging a review for these patients. Ensure patients have the appropriate monitoring tests in place when required.

Risk stratification - identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches.

Service Development - contribute pharmaceutical advice for the development and implementation of new services that have medicinal components (e.g., advice on treatment pathways and patient information leaflets)

Information management - Analyse, interpret and present medicines data to highlight issues and risks to support decision making.

Medicines quality improvement - undertake clinical audits of prescribing in areas directed by the GPs, feedback the results and implement changes in conjunction with the relevant practice team.

Medicines safety - implement changes to medicines that result from MHRA drug safety alerts.

Implementation of local and national guidelines and formulary recommendations - monitor practice prescribing against the local and national formulary. Make recommendations to GPs for medicines that should be prescribed by hospital doctors or subject to shared care. To work with local medicines management team to ensure practice compliance to local formulary.

Education and Training - provide education and training to primary healthcare team on therapeutic and medicines optimisation

Care Quality Commission - Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved

Public Health - To support public health campaigns. To provide specialist knowledge on all public health programmes available to the general public.

Collaborative working arrangements

Participate in the PCN MDT meetings.

Essential Qualifications

Master's degree in Pharmacy (MPharm) (or equivalent).

Mandatory registration with General Pharmaceutical Council (GPhC).

Independent prescriber and completion of CPPE.

Knowledge, Skills and Experience Required

Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council

Minimum of 2 years' experience as a pharmacist

Have some experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice

Recognises priorities when problem-solving and identifies deviations from normal pattern and can refer to seniors or GPs when appropriate

Able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct
