Job Overview

We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for patients. This position requires a strong foundation in anatomy, medication administration, and patient care to provide optimal pharmaceutical services. The role is full time and a mix of remote and practice based working.

Responsibilities

The post requires autonomous working to provide pharmaceutical care to our patients. You will have daily appointments to offer medication reviews and medication advice as well as minor alignments and other contractual obligations including QOF and IIF schemes

Completing medication reconciliations and reviews for patients SMR both in person and over the telephone

Complete SMR

Medicine optimisations and medicine RX

Prescribing medication

Must be able to offer sound and professional advice in relation to clinical management issues

Maintain and improve quality and patient safety

Provide pharmaceutical input for clinical governance meetings

Requirements

A degree in Pharmacy from an accredited institution. Registration with the General Pharmaceutical Council (GPhC) or equivalent body. Strong knowledge of anatomy and physiology relevant to medication management. Experience in medication administration within a clinical setting, preferably in a primary care environment. Excellent communication skills with the ability to work collaboratively within a multidisciplinary team. A commitment to providing high-quality patient care and ensuring safety in medication practices.

Job Type: Full-time

Pay: 21.00-22.50 per hour

Expected hours: 37.50 per week
