As part of our expandingPharmacy Team, you will play a vital role in supporting both patients and practices. The Pharmacy Team is embedded within our broaderAdditional Roles Reimbursement Scheme (ARRS) workforce, which includes a diverse range of professionals working together to enhance patient outcomes. This multidisciplinary approach ensures that our practices are well-supported and that our communities benefit from coordinated, accessible, and compassionate healthcare.
Main duties of the job
To provide excellent patient care, to improve resilience within our member practices, and to support practice teams with expert clinical knowledge of medications. This will be achieved by face-to-face and telephone appointments, project work around differing long-term conditions as well as CVD prevention, and building positive relationships with key stakeholders.

Job responsibilities
Patient-facing long-term conditions clinics -Review the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e. medicines optimisation). Make appropriate recommendations to clinicians for medicine improvement.Request and interpret appropriate monitoring for long-term conditions.
Patient-facing clinical medication review-Undertake clinical medication reviews with patients autonomously.
