Core Responsibilities Undertake medication reviews and Structured Medication Reviews (SMRs), particularly for patients with polypharmacy or frailty. Support long-term condition management by optimising therapy and contributing to chronic disease clinics. Manage repeat prescriptions and resolve medication-related queries from patients and staff. Conduct medicines reconciliation for patients transferring between care settings.
Monitor and manage patients on high-risk medications, ensuring appropriate blood tests and safety checks are completed. Implement medicines safety initiatives and contribute to audits and quality improvement projects. Analyse prescribing data to identify trends, risks, and areas for improvement, supporting safe and effective prescribing decisions. Attend clinical meetings and provide medicines-related advice to the multidisciplinary team.
Support patients to take ownership of their treatment plans through education, self-care strategies, and tailored advice. Promote adherence and understanding of their medicines to improve outcomes and reduce health inequalities. Engage in professional development, including training for independent prescribing if not already qualified. Advanced Responsibilities Independently manage and prescribe for patients with long-term conditions within pharmacist-led clinics, in line with clinical competence and local protocols.
Prescribe autonomously as an Independent Prescriber within clinical competence. Lead on medicines management and prescribing safety initiatives within the practice. Contribute to the development and review of clinical protocols and standard operating procedures to support safe, consistent prescribing and repeat medication processes. Oversee quality improvement and audit projects, using data to drive prescribing improvements.
Mentor and supervise junior pharmacy staff and provide training to the wider clinical team. Develop pharmacy-led clinics and services in collaboration with practice leadership. Provide expert input in complex cases and support decision-making across the MDT. Represent the pharmacy team in meetings and contribute to strategic planning.
Take a lead role in medicines-related risk management and regulatory compliance. Support the practice in meeting CQC standards relating to safe and effective medicines use, and assist in managing medication-related incidents, complaints, and improvement actions.
