Important information about this post

We are looking for a qualified and experienced prescribing pharmacist providing support for patients. The post holder will need to take responsibility for prescription and medication queries, coding of medicines from outside agencies, carry out audits and reviews within the practice including medications with shared responsibly with secondary care.

A proactive attitude, initiative, self-motivation and drive is essential to help reduce the workload of GPs.

Working in accordance with the GPhC Code of Conduct, the post holder will work as part of a multi-disciplinary team to provide expertise in clinical medicines use and optimisation. The post-holder will deliver a high quality, clinical pharmacy service to patients in general practice ensuring the safe, clinically effective and cost-efficient use of drugs, using expertise in independent prescribing to support improved access to medicines.

Qualifications



GPhC Pharmacy degree (MPharm) (essential) CPPE qualification Primary care pharmacy education pathway : CPPE (desirable) Holds an independent prescribing qualification (desirable)

Experience



Has an experience/awareness of the breadth of common acute and long-term conditions that are likely to be seen in general practice Ability to prioritise and problem-solve within remit of role, knowing when to refer onto GPs appropriately Able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct Involves patients in decisions about prescribed medicines and supporting adherence as per national guidelines.

Hours



Negotiable; 25-37.5hrs per week hours to be worked across 5 days ideally. Occasional Saturday morning.

Salary Scale



40,000-48,000 per annum, pro rata. Salary is dependent on experience, skills, prescribing and qualifications

Contract



Permanent (subject to 6 month probationary period)

Uses skills in a range of routine situations requiring analysis or comparison of a range of options Excellent interpersonal, influencing and negotiation skills, organisational skills Computer skills essential and experience of SystmOne desirable Demonstrate personal accountability, emotional resilience and work well under pressure Ability to work across two practices within the PCN if required

Duties and Responsibilities



Carry out medication review consultations, face to face and over the phone. To clinically assess and treat patients using expert knowledge of medicines Authorising and signing repeat prescriptions where qualified Process documentation re medication for patients recently discharged from hospitals or other providers Helping the practice deliver on the IIF, QOF, enhanced services and prescribing incentive scheme targets. Work with lead GP for medications as a contact for medicine-related queries Assist with the management of National Patient Safety Alerts relating to drugs (warnings, recalls, shortages, changes etc) Work collaboratively with the Patient Care Coordinators and Trainee Pharmacy Technician to implement improved processes for the management of prescriptions within the practice and the optimisation of patient care Contribute to ensure streamlining medicines management systems in practice Supervise (Pre-Registration) Pharmacy Technicians and mentor Improve the quality and effectiveness of prescribing through clinical audit and education to improve performance against NICE standards, and clinical and prescribing guidance. Attend prescribing and educational meetings, take notes, obtain most up to date TAG info and feedback to the practice (upon liaising with lead GP) and disseminate info to clinicians and management Ensure the practice and clinicians are adhering to prescribing guidelines, update all at internal meetings as required Assist with CQC searches and shared care monitoring as directed Develop through participation in clinical supervision, training and service redesign activities Keep accurate computer and paper records of all patient contact Adhere to GDPR (data protection) and confidentiality Participate in such other duties and responsibilities as may from time to time be reasonably assigned.
