Job Title: Remote Clinical Pharmacist - Prescriber

Overview:
We are currently seeking a dedicated Clinical Pharmacist (Independent Prescriber) to join our Primary Care Network team on a remote basis (we provide laptop and mobile phone). This position offers an exciting opportunity for individuals with a passion for patient care and medication management.

Duties:
- Action requests for changes in medication and medication reconciliation
- Review patient medical histories and medication regimens to ensure safe and effective drug therapy

- Undertake medication reviews
- Provide drug information and counselling to patients on proper medication use
- Conduct medication reconciliation and ensure accurate documentation of pharmaceutical care interventions

Qualifications:
- Bachelor of Pharmacy degree or equivalent qualification
- Registration as a pharmacist with the General Pharmaceutical Council (GPhC)
- Proven experience in a Primary Care setting, with knowledge of EMIS Web and SystmOne
- Strong understanding of anatomy and its application in patient care
- Excellent communication and interpersonal skills
- Ability to work effectively in a multidisciplinary team
- Proficiency in medication administration procedures
