We are looking to recruit a full time (Mon to Fri) Clinical Pharmacist with General Practice experience to join our Team. You will be part of a clinical team of 7 GPs and a total of 2 pharmacists in an 11,000 patient practice in Notting Hill. We are a telephone triage based team which combines both telephone and a face to face contact. Your role will touch on all aspects of primary care with a focus on acute and repeat prescription, medicines monitoring, chronic disease management, management of minor ailments as well as excellent opportunities for personal professional development. Main Duties:
Prescriptions
: involvement in the issues of acute, repeat and dossette box prescriptions medication, medication reviews, reconciliations.
Clinical correspondence:
processing the daily clinical correspondence from medical colleagues and taking appropriate action in particular medication changes following outpatient appointments or hospital admission.
Patient contact:
both on phone and face to face. Discussing and clarifying medication changes. Clinical assessment for cases of minor ailments
Clinical triage:
being part of a clinical triage team, working next to GP's taking incoming calls from patients and triaging appropriately.
Specialty clinics:
the potential to specialise into the management of chronic disease areas and run annual reviews clinics e.g. diabetes COPD, Asthma and mental health.
Monitoring:
management of safe prescribing in patients with potentially harmful medications e.g. DMARD, warfarin, lithium.
PCN:
contribute to the standards and prescribing aspirations of the local PCN to help standardise and maintain high quality across all practices in the PCN.
Minor ailments:
build up your own portfolio of managing minor aliments e.g. UTI, MAP, chest infection, asthma, COPD etc.
Personal development:
full professional development opportunities through the CPPE primary care pathway and independent prescribing programme. Extensive exposure to everyday clinical practice, close supervision by senior medical and pharmacy team to promote the evolution of a highly competent clinical pharmacist.
Requirements
A degree in Pharmacy from an accredited institution.
Registration with the General Pharmaceutical Council (GPhC).
