Summary

We are seeking a Clinical Pharmacist to join our dynamic team at Ancora Medical Practice in Scunthorpe. In this role, you will play a crucial part in optimizing patient medication management and ensuring safe and effective pharmaceutical care, contributing significantly to our mission of enhancing patient outcomes.

Responsibilities

Conduct comprehensive medication reviews and assessments for patients. Conducting medication reviews and medicines optimisation. Supporting chronic disease management (e.g., diabetes, hypertension, asthma, COPD). Providing advice and support to patients on medication use, side effects, and adherence. Ensure compliance with pharmacy regulations and standards of practice. Support quality improvement initiatives within the practice. Supporting audits, QOF, IIF, and other quality improvement initiatives. Working collaboratively with GPs, nurses, and other healthcare professionals.

Qualifications

Qualified Clinical Pharmacist with GPhC registration. Experience of working in primary care or a similar healthcare setting desirable. Strong communication and interpersonal skills. Ability to work independently and as part of a team.

Pay: 38,374.83-56,942.77 per year
