We are seeking a part-time Clinical Pharmacist (30 hours per week) to join our established team at the Sunderland Central PCN.In this patient-facing role, you will provide expertise in all areas of medicine-related practice while working within your professional and clinical boundaries. You will work in partnership with various organisations to enhance the quality and safety of care offered to our patients. Key responsibilities include: Using independent prescribing to support better access to medicines Clinical medicines management Providing face-to-face structured medication reviews Tackling polypharmacy and managing long-term conditions Managing medicines on transfer of care Implementing systems for safer prescribing Managing repeat prescriptions and acute prescription requests Supporting antibiotic stewardship Addressing both public health and social care needs across PCN practices Ensuring integration with community and hospital pharmacy to utilise skill mix and improve patient outcomes This pivotal role aims to improve the quality of care and operational efficiencies, requiring motivation and passion to deliver an excellent service within general practice.
