An exciting opportunity has arisen for a motivated and experienced Clinical Pharmacist to join our friendly and forward-thinking team at Taunton Vale Healthcare.
We are a high-performing, innovative GP practice committed to continuous improvement, collaborative working, and delivering outstanding patient care. This role offers the opportunity to work across both clinical and operational areas within a supportive multidisciplinary team.
About the Role:
As a Clinical Pharmacist, you will work alongside GPs, nurses, and the wider team to support safe and effective medicines use and contribute to the delivery of long-term condition management, particularly in respiratory and chronic care. You will:
Conduct structured medication reviews in person, via telephone, or within care homes
Be independent prescribing or actively working towards the qualification
Provide expert clinical support for acute and repeat prescriptions, prescription queries, and medication safety
Perform medicines reconciliation following hospital discharge and manage high-risk medicines
Identify and support patients at risk from medicines-related harm
Support practice compliance with CQC and public health initiatives
Monitor prescribing practices against local and national guidelines including Somerset Formulary and NICE guidance
Contribute to QOF and Enhanced Services performance
Deliver education and training to the primary care team and supervise Foundation/IP Pharmacists
Analyse prescribing data to inform quality improvement and service development initiatives
Work collaboratively across community and hospital pharmacy to improve continuity of care and manage workload effectively
What We're Looking For:
GPhC-registered pharmacist with at least 3 years' post-qualification experience
Experience in general practice or primary care desirable
Independent Prescribing qualification (or committed to obtaining it)
Strong knowledge of long-term conditions, particularly respiratory disease
Confident in medicines optimisation, clinical audit, and service improvement
Skilled in using clinical systems, data analysis tools, and working within QOF and DSQS frameworks
Able to educate, mentor, and supervise junior pharmacists or students
Passionate about improving patient care, reducing medicines-related risk, and shaping the future of pharmacy in general practice
