Coastal PCN are looking for an experienced and motivated Clinical Pharmacist to join our growing medicines management team. This is an exciting opportunity for a forward-looking professional who wants the opportunity to play a key role evolving medicines management within our PCN. Our Clinical Pharmacists and Pharmacy Technicians are highly regarded and integrated team members that are well supported by the wider PCN.
Our Primary Care Network (PCN) has a patient population of over 29,000 people set in beautiful mid Cornwall, incorporating 4 local GP practices (St. Agnes, Perranporth, Carnon Downs & Chacewater), which have excellent inter-relations and a long history of working positively together. Our population is diverse compared to other areas in Cornwall, with varying levels of socio-economic status, young families and elderly patients all of whom make providing care interesting and rewarding.
Responsibilities and Duties
As one of our Clinical Pharmacists, you will work as a key part of a multi-disciplinary team supporting, promoting and implementing high quality, evidence based, cost effective and safe use of medicines. You will use your clinical medicines expertise to support both patients and general practice teams and improve prescribing practices across the PCN through mechanisms such as process reviews and clinical audits. You will conduct structured medication reviews to proactively manage patients with complex polypharmacy and manage the medicines aspects of chronic illnesses such as hypertension and heart failure. As part of a collaborative team, you will work with other pharmacy colleagues across the PCN, including the ICB Pharmacy Team to ensure an effective and integrated approach to medicines optimisation for our population.
You will:
Perform medication reviews for patients face to face and over the telephone.
Liaise with the Practice Clinical team on prescribing and medication issues, especially around management of long-term conditions.
Support the processing of complex repeat prescribing medication requests made by patients or other repeat prescribing queries.
Support patients through structured medication reviews, to take their medications and get the best from them, reduce waste and promote self-care.
Discuss complex and sensitive information with patients including medical histories, lifestyles, and personal circumstances and to refer on to other services and professionals as appropriate.
Provide leadership and supervision for the on-site PCN Pharm Techs
Provide leadership on person-centred medicines optimisation and quality improvement whilst contributing to the quality and outcomes framework and enhanced services.
Provide general practitioners with appropriate recommendations and action plans following reviews with patients to optimise treatment with medicines and ensure appropriate follow up.
Record medication review plans, recommendations and outcomes on GP clinical systems and own clinical notes in accordance with practice procedures.
Reconcile medicines following discharge from hospitals, intermediate care and into care homes; identify and rectify unexplained changes; manage changes by referral to the GP; perform clinical medication review; support the production of post-discharge medicines care plans and work with patients and other pharmacy colleagues to ensure patients receive the medicines they need.
Develop as an independent prescriber to manage and prescribe for patients within a specific chronic disease area and within own areas of competence.
With appropriate training, monitor the effects of medication by checking clinical signs (e.g., BP, pulse, etc).
Assist with delivery of enhanced health in care homes initiatives across the PCN.
Other Duties:
As part of the multi-disciplinary team, attend relevant MDT meetings, providing information and feedback.
Report medicines related incidents, contribute to investigations and root cause analyses as well as participating in serious incident investigations and multidisciplinary case reviews.
Deliver training, mentoring and guidance to other clinicians and staff on medicine issues.
Ensure that, within each practice you work, staff and clinicians are kept up to date regarding changes in national and local medicines guidance and legislation.
Critically appraise clinical evidence and present information to prescribers via prescribing meetings and educational events, as required.
Lead on medicines management governance arrangements to adhere to CQC standards, including assisting in developing policies and SOPs.
Review repeat prescribing protocols and processes and support staff in their implementation.
Perform prescribing audits and direct/supervise the audit activity of others.
Facilitate good working relationships between practices and local community pharmacists.
Be responsible for delivering local prescribing quality initiatives in collaboration with ICB medicine management.
Qualifications and Skills
Essential
Master's degree in pharmacy (MPharm) Mandatory registration with General Pharmaceutical Council
Minimum of 2 years post-qualification experience.
A minimum of 12 month's experience in either inpatient Clinical Pharmacy or in Primary Care.
In depth therapeutic and clinical knowledge and understanding of the principles of evidence- based healthcare.
An appreciation of the nature of general practice and of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (i.e. patients).
Able to plan, manage, monitor, advise and review general medicine optimisation.
Able to obtain and analyse complex technical information.
Recognises priorities when problem solving and identifies deviations from the normal pattern and able to refer when appropriate.
Ability to help meet EA targets as required by the PCN.
Produce timely and informative reports.
Demonstrates accountability for delivering professional expertise and direct service provision.
Desirable
Qualification in Independent Prescribing via an approved 18-month training pathway such as the CPPE Clinical Pharmacist training pathway.
