Trinity Health Group PCN is looking for a Clinical Pharmacist to join our current team of Clinical Pharmacist and Pharmacy Technicians. The post holder will work alongside the existing team to support meeting the needs of our patients.
Main duties of the job
The post holder will be undertaking structured medication reviews, improving medicine optimisation and safety, supporting care homes, as well as running practice clinics, as set out in the PCN DES. Part of this role will also include working within our Pharmacy Hub alongside the Pharmacy Team to provide a safe and effective Pharmacy Service. To provide professional specialist medicines advice to GPs, Nurse Practitioners, Practice Nurses, Community Pharmacists, Community Nurses, and other health professionals to promote the safe, effective, and high quality, cost-effective use of medicines in all prescribing decisions for the benefit of the Practice's patients. You will also be actioning acute prescription requests and conducting medicines reconciliation.

Job responsibilities
The following are the core responsibilities of the PCN Clinical Pharmacist. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
The post holder will:
Undertake level 3 medication reviews to support improved patient outcomes.
Undertake Structured medication reviews for patients who are highlighted in the DES.
Review and issue Acute medicine requests.
Carry out Medicines reconciliation of hospital discharge/clinic letters.
Improve medicines optimisation.
Support Practices to achieve QOF targets as required.
To provide advice to GPs, Nurse Practitioners, Practice Nurses, Community Pharmacists, Community Nurses, and other health professionals to promote the safe, effective, high-quality, cost-effective use of medicines in all prescribing decisions for the benefit of the Practice's patients.
Contribute to the improvement of health outcomes in long-term conditions as part of the multidisciplinary team.
To provide prescribing advice and support to the practice(s), on all medication issues.
Accurate and consistent recording and monitoring of data including incident reporting, prescribing data, and patient outcomes.
Work in partnership with GP practices and the ICB /ICS
Ensure that all major MHRA drug alerts, where appropriate and within levels of competence, are acted upon within practices over an appropriate time frame and advise on any medication changes that occur as a result as instructed by the ICB Medicines safety Officer.
Provide peer support to other clinical pharmacists and pharmacy technicians in the PCN.

Master's degree in pharmacy or equivalent
Registration with the General Pharmaceutical Council (GPhC)
Evidence of Continued Professional Development
Independent Prescriber or willing to work towards
Completion of the Primary care pharmacy education (CPPE) pathway or willing to work towards.

Postgraduate Diploma in Clinical Pharmacy (or equivalent)
