About the Role
We are looking for a
Clinical Pharmacist
to join the White Practice (Walton Practices Federation PCN). In this patient-facing role, you will work as part of a multidisciplinary team to optimise medicines use, manage chronic diseases, and improve patient care. This is a great opportunity for a pharmacist passionate about delivering high-quality healthcare and improving patient outcomes.
Key Responsibilities
Conduct structured medication reviews to optimise patient care, focusing on reducing polypharmacy, inappropriate use of antibiotics, and over-medication.
Support patients with chronic diseases (e.g., asthma, COPD, frailty, care home residents, and complex multi-morbidity).
Collaborate with GPs and healthcare teams to ensure patients receive the best outcomes from their medications.
Lead on medicines safety and quality improvement initiatives, including tackling inequalities in healthcare.
Helping patients manage their medications and improve adherence.
Work closely with secondary care, community pharmacy, and other healthcare professionals to ensure seamless care and better patient outcomes.
Provide medicines optimisation and work under direct supervision of GP Principal of the White Practice to develop Face to Face consultation, auditing skills and safe prescribing practices.
Person Specification
Qualifications:
M.Pharm degree, registration with the General Pharmaceutical Council (GPhC).
Independent Prescribing qualification (IP ) and CPPE course are advantages.
Experience:
Significant post-qualification experience in primary care or hospital settings is preferable.
Skills:
Strong clinical knowledge, excellent communication skills, time management, and problem-solving abilities.
Desirable:
Experience in primary care pharmacy, the ability to manage chronic conditions, and experience with prescribing and medication reviews.
Working from site (Walton Health centre) is desirable.
