An opportunity has arisen at The Elms for a Prescribing Clinical Pharmacist to join our family friendly, patient centred, forward thinking practice based in the centre of Watford. We are looking for an enthusiastic, experienced pro-active lead Prescribing Clinical Pharmacist.

The ideal candidate will play a crucial role in optimising patient care through effective medication management and consultation. You will work closely within our team and ICB colleagues to ensure the safe and effective use of medications

Main duties

Support the practice and its patients Review and interpret medication requests, ensuring accuracy and appropriateness for each patient's condition. Collaborate with GP's, PA's and nursing staff to develop comprehensive medication plans tailored to individual patient needs. Medication and long term condition reviews Repeat prescription re-authorisation Processing of hospital discharge letters Regular Medicines Audits Support QOF and ECF targets Steer development of a prescribing team

Experience

Bachelor's Degree in Pharmacy A GPhC registration holder who has completed a CPPE pathway, such as the Primary Care Pharmacy Education Pathway (PCPEP) or the General Practice Pharmacist Training Pathway (GPPTP). Independent prescribing qualification More than 2 years previous experience in a GP practice

Job Types: Full-time, Part-time, Permanent

Pay: 48,750.00-57,000.00 per year

Expected hours: 20 - 30 per week
