Main duties of the job
We are seeking a motivated Clinical Pharmacist (IP) to work alongside our GPs, nurses, HCAs, and admin team to improve patient care and medicines optimisation. This is a patient-facing role based at the practice, with responsibilities including:

  • Conducting structured medication reviews and managing long-term conditions.
  • Prescribing safely and effectively within scope of practice.
  • Supporting repeat prescriptions, hospital discharges, and medication queries.
  • Driving medicines safety, cost-effective prescribing, and QOF-related projects.

Key Duties and Responsibilities
  • Medicines Management & Optimisation
  • Undertake medication reviews (face-to-face and telephone).
  • Manage repeat prescriptions and reauthorisations.
  • Support the implementation of local and national prescribing initiatives.
  • Monitor high-risk medications and ensure appropriate blood test monitoring is completed.
  • Support cost-effective prescribing and adherence to formulary guidance.
  • Clinical Support
  • Provide structured medication reviews for patients with long-term conditions (e.g. hypertension, diabetes, COPD).
  • Support hospital discharge summaries, clinic letters, and medication reconciliation.
  • Offer advice on minor ailments within competence and support self-care strategies.
  • Work collaboratively with GPs and practice nurses to optimise patient pathways.

  • Support patients to improve adherence and understanding of their medicines.
  • Provide lifestyle and health promotion advice where appropriate.
  • Signpost patients to community pharmacy, secondary care, or other relevant services as needed.

  • Ensure prescribing is safe and in line with NICE, local, and national guidance.
  • Contribute to audits, quality improvement projects, and practice clinical governance.
  • Support CQC compliance by ensuring robust medicines management systems.

  • Participate in daily debriefs, weekly clinical meetings, and multidisciplinary team discussions.
  • Contribute to staff training and patient education initiatives.
  • Support the training practice ethos by mentoring junior colleagues, students, or trainees where appropriate.
