Job Description

To have and continue to develop advanced skills in patient assessment, making appropriate clinical decisions and ensuring patient safety Provide clear and effective documentation, completing the Electronic Patient Record fully. Effectively manage the deteriorating patient outside of Intensive Care areas, facilitating transfer into Intensive Care as necessary Support patients prior to discharge from Intensive Care and High Dependency Unit and monitor their progress on the ward. To provide a high level of clinical advice and expertise to ward staff in relation to children who have recently been discharged from intensive care or those who have the potential to deteriorate. A member of the Clinical Emergency Team (Resuscitation Team) and to have advanced resuscitation skills Responsibility for supporting ward staff during an emergency event and facilitating debrief following the event.



Able to escort patients and critically ill patients to other clinical areas within the Trust. Respond to patients with PEWS and Sepsis triggers supporting ward nurses in escalating and responding to clinically deteriorating patients in line with the RAPID policy. Assist the Senior Clinical Site Practitioners and medical staff with patients that are flagged by the sepsis screening pathway, helping to make clinical diagnosis, and referring to other health professionals as require Provide expertise to ward staff in relation to intravenous drug therapy including peripheral and central line management. Other areas of knowledge and delivery of care will include care of tracheostomy, suctioning, long-term ventilated patients Undertake cannulation and venepuncture when and where necessary, whilst supporting nurses in the acquisition of these skills Advise nursing and clinical staff regarding drug availability and appropriate usage and refer to pharmacist as appropriate.



Work in conjunction with Infection and Prevention Team ensuring all policies and procedures are followed and patients are placed and cared for in the correct environment.
