Job overview




We are looking to recruit a Clinical Skills Practitioner for the SMI Suffolk Physical Health Team, a team of health professionals focused on the delivery of our Physical Health agenda. The team's primary focus is to:




Deliver a reduction in physical health illness in those service users across Suffolk both in primary and secondary care, with severe mental illness (SMI).



Undertake physical health screening for service users with complex health needs, in particular, the Annual SMI Physical Health check.



Flexibility in supporting the provision of physical health checks for patients who attend the Suffolk Adult Eating Disorder Team, and patients who attend Suffolk Clozaril Service clinics.



Ensure that the physical health needs of the service users are met through sharing healthcare information with the service users' primary care team and other stakeholder.



Promote service user self-management, self-care, recovery and the maintenance of independence.



Improve communication between service users, professionals and carers.



Ensure a high level of service user and professional satisfaction.



Develop and maintain successful, collaborative relationships with both primary and secondary care services.




Flexibility in meeting the requirements of the service.

Main duties of the job




As part of the key frontline staff for our expanding service, you will be carrying out physical health assessments and monitoring for patients with severe mental illness (SMI) as well as, on occasion, flexibly support physical health assessments for patients with mild to moderate eating disorders, and those attending the Suffolk Clozaril Service, as needed.





You will be supporting patients to meaningfully develop recovery strategies that promote their physical and mental wellbeing, a healthier lifestyle, optimal independence and self-care as well as promoting the involvement of families and carers in care, reflecting their diversity, equality and rights to ensure their needs are met. This will be carried out alongside the service user's primary care team as well as care coordinator/lead professional where applicable.





You will ideally possess a level 4 Foundation Degree or NVQ 4 in a health field or as a minimum hold an NVQ 3 in care/health or equivalent. You will be able to evidence your knowledge of skilful patient care delivery and demonstrate excellent interpersonal and communication skills alongside your skills in planning and prioritising your own workload.

Care certificate or completed within first 12 weeks of start date, or relevant equivalent qualification. Diploma in healthcare with clinical experience, or relevant equivalent qualification #####
