37.5 hrs per week

Responsibilities:

We are looking for an enthusiastic and highly motivated individual who has a passion for delivering high quality and seamless nursing care to patients in their own homes and in clinic settings, being proactive in coordinating their care needs as appropriate.




Working within the

Community Nursing Network (CNN) Tissue Viability Team

, you will undertake duties involving delivery of nursing services and holistic assessments of patient's health needs using a person-centred approach.


The role will have a specific focus on the management of patients with lower limb wounds, including supporting the community nursing teams in delivering the national agenda in particular in relation to lower leg ulcer management, performing assessments and creating management plans and supporting in the community-based leg ulcer clinics.


The post holder will act as an expert nurse and provide advice, intervention and training to a patient group, promote a seamless service within a multi-professional framework and will contribute to the development of the speciality service.


In addition, you will have the opportunity to be involved in facilitating self-care for patients with long term conditions, health promotion and innovative activities and projects that enhance patient service delivery.


Car driving essentials for this Role is you will need to be prepared to travel across Kirklees to work in clinics and visit patients in their own homes.


The service is able to offer some flexibility with working hours in regard to carer responsibilities and work life balance; shifts times can be negotiated as long as the service needs are met.
